(No more than 6 months old) Can be taken at: AAA, CVS, Walgreens, UPS Store on Commerce Way or look in the Yellow Pages under "Passport Photos" (or Phone Book Online = http://www.superpages.com ). - Proof of Citizenship:
Certified Birth Certificate (certificate has to have raised seal and the day it was recorded with the Town you were born in) OR Expired Passport.
- Proof of Identity:
Drivers License OR Card that has a description & photo that is a good likeness of you with your signature.
- Checks or Money Order Only The Town of Seekonk does not accept debit or credit cards
RENEWALS:
Passports issued less than 15 years ago and have expired can be renewed using a special mail-in
form. Form DS-82 can be downloaded from the Department of State, travel.state.gov. Follow the instructions on the website.
Cost for applicants 16 years and older:
$100.00 no matter where you apply.
Have a money order or check made payable to U.S. Dept of State for $75.00.
The remaining $25.00 is an application fee that can be paid by cash or
a check made payable to Town of Seekonk. Cost for applicants 15 years and younger:
$85.00 no matter where you apply.

- You will receive the passport within 6 - 8 WEEKS.
- Passports can be expedited for an additional $60 and received in 3 - 4 WEEKS

PASSPORT TIMELINE - By January 1, 2008, U.S. citizens traveling between the U.S. and Canada, Mexico, Central and South America, the Caribbean and Bermuda by LAND OR SEA (including ferries) may be required to present a valid U.S. passport or other documents as determined by the Department of Homeland Security. While recent legislative changes permit a later deadline, the Departments of State and Homeland Security are working to meet all requirements as soon as possible. Ample advance notice will be provided to enable the public to obtain passports or passport cards for land/sea entries.
